Application for Form 1 Sek Men Integrasi Sains Tahfiz (SMISTA) MAIWP 2012

Application are invited from eligible candidates to pursue form 1 at SEKOLAH MENENGAH INTEGRASI SAINS TAHFIZ (SMISTA) MAIWP 2012

Eligibility

  • Obtained 5A in Ujian Pencapaian Sekolah Rendah (UPSR).
  • Obtained 5A in Ujian Penilaian Sekolah Rendah Agama (UPSRA)
  • Posses basic Al-Quran studies (i.e tilawah dan hafazan)
  • Borned in Wilayah Persekutuan. However, 10% of the allocation will be opened to candidates who are borned outside wilayah persekutuan.

How to get application form

You can obtain the application form by the following method :

  • Download application form at MAIWP http://www.maiwp.gov.my/
  • Collect at SMISTA-MAIWP, Bandar Tun Razak, Cheras.
  • Collect at Kaunter Pertanyaan Tingkat 7, Bangunan PERKIM, Jalan Ipoh, Kuala Lumpur.

How to apply

Completed application form must be sent to :

Ketua Pegawai Eksekutif,
Majlis Agama Islam Wilayah Persekutuan,
Aras 7, Bangunan PERKIM, No 150, Jln Ipoh,
51200 Kuala Lumpur.
